Disposition of Inactive Special Education Records
The Blue Folder Destruction Form is in Portable Document Format (pdf) and in Microsoft Excel format (xls).
In order to view and print the form in pdf, click the pdf link below. Complete the needed information, sign and hand carry with the blue folders to the district office on the designated destruction day. Make sure to keep a copy of the completed form for your records.
In order to submit the information electronically, click the excel link below. Once completed, rename and save the form. Send as an attachment to Tracy, District Special Programs secretary. Print two copies of the completed form, sign and hand carry the blue folders to the district office on the designated destruction day. Make sure to keep a copy of the completed form for your records.
