Student Travel Policy and Forms

Provo City School District has developed policies and procedures pertaining to student travel and field trips. These policies and procedures contain four parts: (1) the District Travel Policy, (2) the Request for Student Travel form, (3) the Authorization to Transport Students form, and (4) the Release Form.

These four documents are available below in PDF format with an explanation on their use and the procedure for submitting the forms for District approval.

District Travel Policy

Please study carefully prior to beginning the Request for Student Travel form.

Request for Student Travel Form

After downloading, this form can be completed electronically with your computer using Adobe Acrobat or Adobe Reader. If you use Adobe Reader, you must print the form after filling it out - you cannot save it. If you wish to save an electronic file of the completed form, you must use Adobe Acrobat (either standard or professional edition).

Open the form, and tab through the necessary spaces that need to be checked or filled in. When you finish completing the form at the top of page 3, print the form. Sign and date the printed form, then submit the form to your Principal for his/her dated signature. The Principal must then submit the signed form to the Superintendent for his approval. The Superintendent will return the form back to you, which may include any special conditions that pertain to your request.

Authorization to Transport Students form

This form must be provided if students are to be transported via privately owned vehicle. Download and complete form electronically with either Adobe Acrobat or Adobe Reader. This form must be signed and dated by the driver and Principal, and must be submitted to the supervisor of the activity.

Release Form

Download and print sufficient copies so that it can be filled in and signed by each student and their parent/guardian. This form should be completed by hand, preferably in ink. A completed and signed Release Form for every student traveling must be submitted to the supervisor of the activity.
